
SERVICES
Our interdisciplinary team consists of clinical psychologists, physiotherapists, medical doctors, pharmacists, occupational therapists, dietician, social workers and other allied health providers who collaborate to provide evidence-based and holistic approaches to pain management. We aim to empower individuals, foster a positive community, and improve the overall well-being of those dealing with persistent pain.

SERVICES ON OFFER INCLUDE:
ACC Pain Triage Assessment
1hr Assessment + IDT Planning
A preliminary assessment of the patient to clarify the nature of treatment and rehabilitation required. Includes an evaluation of the causes and contributors to pain. Typically completed by physiotherapist and reviewed in an interdisciplinary team including medical doctor, psychologist, and other allied health as indicated. If
required further referrals to specialist pain medicine physicians and/or interventional procedures can be facilitated.
Upon completion a summarized report will be sent back to the referrer and ACC.

Pain Group Education Program
6 x 2hr in person group sessions
A specialized, stand-alone program for patients who would benefit from education about pain and learning
self-management strategies.

ACC funded Community Support
6-12 weeks of input 2-3x/week
The purpose of community services is to support patients to develop self-management strategies aimed at improving their quality of life, return to participation in daily activities, and/or work. Interventions
are tailored to each patient, with access to input from relevant professional disciplines.
A psychological skills group provides additional complementary skills for managing mood, based on CBT, ACT, and mindfulness therapies.
